The BDM office reserves the right to request for any additional documents depending on the unique circumstances of any particular case.
Requirements for Reprint of Certificates (BC, MC and DC)
- Completed Request form
- Valid photo ID ( FRCS/FNPF/Voter ID/DriversLicense/Passport)
- Applicant must sign the request form
- Letter of authority and valid ID from the person whose BC or MC is required (if printing on behalf)
- If you are residing overseas (Foreigners/Citizens), kindly liaise with the Fiji High Commission office of your convenience.
- For Death Certificate- Letter of Authority and valid ID from next of kin, Executor/Trustee under the probate jurisdiction or Administrator under the letter of administration
